New Patient Registration

The practice welcomes patients for registration regardless of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation.

We may only register patients within our Practice Area, please see the tab above.

The practice requires a registration form (GMS1) to be completed and signed for each new patient, to provide details of name, address, date of birth, previous address and doctor, and National Health Number (if known).

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
